Fertility Alternatives, Inc. Egg Donor Program
| I would like to offer my sincerest thanks and appreciation to all of the egg donors who have listed with my program. In order to thank you for exclusively listing with Fertility Alternatives, Inc., a referral incentive program has been developed for you. This program is a great opportunity for all the egg donors in my program to receive compensation even if they haven't been matched with a recipient, yet. You are encouraged to offer this information to the donors you referral as well, so they can share the same opportunity. For every potential donor you refer to Fertility Alternatives,Inc. Egg Donor Program, the following incentives may apply. |
| Incentives are paid for every cycle the referred donor completes. The referred donor will still receive her full compensation listed with the agency. Incentives are as follows: The amount received by the Referrer is $300 per cycle completed by the Referred donor. |
General Rules and conditions of the Referral Incentive
Program.
In order to qualify for the incentives above, the following
must apply.
1. You must be a currently enrolled exclusively with Fertility
Alternatives, Inc. You must not be listed with any other agency or
clinic program nor be trying to match yourself independently. If
you have already reached your cycle limit but have worked with
Fertility Alternatives, Inc. for past cycles, you still qualify for the
incentive program.
2. Each donor "referrer" and donor "referral", must complete a
referral form/contract.

3. The potential donor you refer, must contact the agency to
receive an application. This can be either by phone, email or from
the website. The referred donor must initiate and establish contact
with the agency.
4.The potential donor you refer, must qualify and be accepted into
the program.
5. The potential donor you refer, must be matched and complete
the cycle with Fertility Alternatives, Inc. before any incentive
payment will be made.
6. Referrers found on other donor lists or listed independently, will
not be eligible for incentives.
