Anticipated Fees for Egg Donation
AGENCY FEES: As of 4/1/07

Profile Fee:
$25.00  per donor profile.

This fee covers time and expense incurred by Fertility Alternatives, Inc. to
provide the egg donor profiles to potential recipients in the event they decide
not to proceed.

Only ONE profile fee will applied to the final installment of the agency fee if
you choose to move forward with a donor in the program.

Basic Agency Fee:
$3,000 or the equivalent of 50% of the egg donor's fee. (whichever is
higher) Payable in two (2) installments.

This fee is in addition to the Donor's fee and is payable regardless of the
amount of the Egg Donor's fee.

(Example: Donor's fee is $8000. Fertility Alternatives' fee is $4000 plus any
additional expenses included in this fee schedule.)
-Basic agency fee will not exceed $6,500.-

If Fertility Alternatives, Inc. is hired to advertise or search for a specific
type of donor required by the Intended Parents, a non-refundable retainer
of $500.00 will be required plus the actual cost of advertising per every
three months of advertising for the Intended Parents. The initial retainer
will be applied toward the final agency fee
for the donor who was secured
as a result of the advertising
.


Trust Account: (Required)
No Charge.

Trust account provided to pay Egg Donor's fee and other typical expenses
from Trust account.
(Bank fees and check charges may apply.)

Trust is to be funded in three installments.

1. The first installment of $2500 will be due with the finalized agency service
agreement, after the Egg Donor has been formally chosen by the Intended
Parents.

2. The 2nd installment will be due upon the signing of the "Egg Donor
Contract" between the egg donor and the Intended Parents.

3. The final installment will be due on or before the day the Donor
administers injectable medications.

Additional Facilitation Fee: $250

This fee is required if it is necessary for Fertility Alternatives, Inc. to pay for
additional expenses from the trust such as medications, Donor labs, travel
expenses, and to make and secure travel arrangements for the Donor and her
companion.

(Bank fees and check charges may apply, and the trust account may need to
remain open for a longer period of time, to cover all bills due for the cycle.)

Third-Party Administrative Fee: $250

This fee is required to compensate the agency for additional time and
expense involved in the coordination of a cycle where the donor's monitoring
is being provided by a different clinic not associated with the main clinic.
This scenario is common where recipients have chosen a donor outside of
their local area.


Health Insurance Policy: $500.00

This fee is payable to Fertility Alternatives, Inc. to cover a mandatory egg
donor accident insurance policy for the donor though AIG. This covers the
donor, recipient, and the administrative costs involved, and will be paid
directly from the Trust account.

(If your clinic provides egg donor insurance, Fertility Alternatives, Inc., is
required to take precedence over the clinic to provide such coverage.)

International Clients: $750.00
Administrative fee per Egg Donor cycle. This fee is waived, if
communications can be arranged by email.


EGG DONOR FEES
and Expenses:

$5,000+* (Depending on needed ethnic background, geographical location,
experience, education, and resulting pregnancies from past donations.)

$5,000-10,000+ For special request,"Exceptional Donors"
(i.e. Ivy league educated, college educated, proven intelligence, etc)

The average fee for "Exceptional Donors" is about $8,000-$10,000
+.

"High-demand", Experienced/Proven, Exceptional Donors, may have
requested fees
significantly higher than the standard.




Travel Expenses:

$100-170 per night for hotel, if necessary.

(donor actual net lost wages up to $100 per day.)

PROFESSIONAL FEES AND EXPENSES:

Psychological Screening: Up to $500.00 for initial screening.
Any special requests such as I.Q testing will be additional. Some Egg Donors
have already had a psychological screening.*  This payment will be placed in
the Trust account for payment by the agency.

Medical Screening: $1500-$2200

For blood work, labs, ultrasounds, and office visits as required by the main
clinic for Egg Donor and partner screening. (Any special requests such as
genetic testing will be additional.)*
This expense may be payable directly to the
main clinic, or may be placed in
the Trust account for payment by the agency to the monitoring clinic for
screening.
New FDA regulations require donor to have additional infectious disease
screening within 30 days of egg retrieval procedure.

Attorneys' Fees: $800-$1100

Legal representation for Egg Donor and Intended Parents.*
Egg donor's attorneys fee will range $300-$500 and will be placed in the
Trust account for payment by the agency.
Intended Parents will be required to pay their attorney directly when they
retain his/her services. ($500-$850)

Satellite Donor Monitoring: $1200-$2000

This covers the cost of the local IVF clinic who will provide the egg donor
monitoring before she travels to your main IVF clinic for final monitoring
and egg retrieval procedure.
(This is for Long distance travelling donors only.)


ADDITIONAL EXPENSES:
Intended Parents shall be responsible for all expenses incurred by the egg
donor on behalf of the egg donation process. (i.e. Medications, medical,
laboratory, psychological, escrow/trust account, and attorney's fees, postage,
wire and/or bank fees,  insurance co-payments or deductibles, etc.)

Other expenses may arise such as egg donor's lost wages or childcare,
excessive mileage at 40 cents per mile, and medical attention due to Ovarian
Hyper-stimulation Syndrome, etc.

Should egg donor have to travel, all travel, lodging, meal allowance and
childcare for the
Egg Donor and a companion will be the responsibility of the
Intended Parents and is industry standard.

Each egg donor has her own individual circumstances that may cause
additional expense or a higher fee. The final result will be determined after
negotiations by all Parties, their attorneys, and Fertility Alternatives, Inc.

Additional expenses such as may also apply.

Professionals' Fees stated above, are only an average. Some professionals'
fees may be higher or lower. Actual fees charged by professionals can be
determined by contacting them directly.

IVF and medication costs are not included in this fee schedule, because these
expenses are typically paid for by the Intended Parents directly to their clinic
and pharmacy.

Disclaimer: Fertility Alternatives, Inc. can only make a rough estimate of the
costs involved in a Donor arrangement. The Donor's individual requests,
circumstances, and each individual professional fee, may drastically change
the financial outcome originally anticipated in a Donor arrangement. This fee
schedule can only give you a better idea of what to expect.
