| If you strongly believe you fit the qualifications of the Fertility Alternatives, Inc., Egg Donor program, click the Link below. 1.Print the application. (PDF File) 2. Fill out all questions in the application by hand thoroughly. Pay special attention to details, spelling and grammar. Typing the profile is okay, if necessary. Please avoid short simple answers to the questions. Your profile will be read by potential recipients. You want them to know why you could be their ideal egg donor candidate. 3. Sign all required pages 4. Mail back to the address on the application along with a copy of your driver's license (or legal photo ID), and 3-5 reasonably attractive, close ups of your face. (They will be returned.) Good photo examples: Head shots Candid close ups (frontal and profile view if possible) Family photos that include you. Any photo where your facial features are completely distinguishable. Some full body photos are helpful. Do not send: Laser copies Pictures of your children (if you are clearly in the photo, then it is okay.) Distant or unclear photos of yourself You will be notified if I need additional family photos. 5. If you believe you qualify for Exceptional Donor Status, please send with your application one or more of the following: Copies of College transcripts (unofficial okay) SAT/ACT scores IQ test results, etc If you need time to acquire this documentation, send the application and pictures first, and mail other documents when you receive them. DO NOT SEND APPLICATION WITHOUT APPROPRIATE PICTURES ENCLOSED OR EMAILED. Do not email photos if you are unable or do not intend to apply with the program.
